What the letter says
Something on the W-7 is incomplete: a document not certified by the issuing agency, a missing return attachment, an unsigned form, or a reason code that does not match the return.
What to do
Send exactly what is listed by the date on the notice. If the notice asks for original or certified documents, use the issuing agency's certification or a certifying acceptance agent rather than mailing a passport.

The application is rejected (CP567) and you start over.
